Case Management

As a highly skilled paediatric physiotherapist, I accept instruction for babies, children and young people during their litigation process, and for their physiotherapy needs post settlement.

Experience

I have over 24 years of experience in paediatric physiotherapy across a broad range of settings, both in the NHS and in the private sector. In the NHS I have had the privilege of working in renowned tertiary centres including Great Ormond Street Hospital, The Middlesex Adolescent Unit, and Barts and The London NHS Trust.  I have extensive experience of working in a Community Team. This has included being a Clinical Lead for a large community paediatric physiotherapy team in London. Other relevant experience includes working as a physiotherapy team lead in a school for children with complex disabilities, being a hydrotherapy lead for a large paediatric physiotherapy team, running an MDT orthotics clinic and working in consultant led orthopaedic clinics.

I have expertise in the assessment and treatment of children with complex disabilities and long term conditions, including cerebral palsy. I am advanced Bobath trained and NDT trained, and have considerable experience of treating babies, children and young people with very mild to very complex presentations in various community settings (homes, nurseries, mainstream schools and special needs schools). I am confident in the use of 24 hour postural management to compliment and augment functional and postural goals.

I assess and treat each child in a warm and friendly manner, and incorporate child and  family centred care to provide holistic and targeted intervention. I place the needs of the child at the centre of their care and understand that a harmonious client-therapist relationship is crucial to effective treatment outcomes.

I am able to offer personalized, goal focused packages of care based on the individual needs of the child.

Assessment

My initial assessment usually takes up to 2 hours and is a comprehensive assessment that includes a detailed medical history and physical examination, with the use of objective markers. The assessment includes a musculoskeletal and postural assessment, as well as a functional assessment. During the assessment, I will collaborate with the child and their family to determine their individual and functional SMART goals.

Following this assessment, I will provide a detailed and timely report which will advise on present and future physiotherapy needs of the child. I will outline the recommended treatment plan and goals, and provide a breakdown of costs.

Treatment

Recommended treatment may include equipment and orthotic recommendations, dry-land physiotherapy to promote development and aquatic therapy. Following instruction, treatment sessions will commence, and I will work with the child and their family to achieve their personal functional goals.

Report Writing Services

I provide reliable and timely reports, which includes Education Health and Care Plans, Annual Reviews, Assessment Reports and Reports for Social Care

Meetings

I am able to attend Multi-disciplinary Team Meetings, Annual reviews, Case Conferences, Child in Need Meetings and other meetings necessary to optimize a child’s care and treatment outcomes.

Joint Working

Where appropriate and agreed, I work with schools and NHS professionals. This could include attending Orthopaedic or Paediatric review appointments; or liaison with teachers, SEN leads, NHS therapists and private therapists. I may request attendance at appointments where my input may be beneficial, such as for consideration of Botox or orthopaedic intervention. Where appropriate, I may write a report for a specific appointment or liaise with professionals and parents on the phone during the appointment.

Training

Part of my intervention always includes training and equipping  parents and care givers with the skills and confidence to meet their child’s physiotherapy related needs at home.

I am also able to provide training at the school for Teachers, Teaching Assistants and Support Staff involved in the care of the child.

Communication

I develop good working relationships with my clients, their families and their wider support network. I am committed to effective and efficient communication that is appropriate, evidence based and empathic. I provide timely responses to case managers, families and carers, and I initiate communication that will improve outcomes for the child.

I work with verbal and non-verbal clients and am familiar with alternative and augmentative communication strategies.
